INSURANCE LICENSING COURSE
Requirements for Life & Health, Property and Casualty
Insurance License and General Testing Information
1. If you are a resident of Arizona and not currently licensed as an insurance professional you must pass the appropriate exams. Examinations are administered by Thomson Prometric. Passing grade of 70% required on each exam.
Exams:
Life & Health 150 Questions - 2.5 hour time limit Fee: $75
Property & Casualty 150 Questions - 2.5 hour time limit Fee: $75
2. Registration and Scheduling of examinations with Thomson Prometric:

MAIL: Mail the completed exam registration form to Thomson Prometric,
ATTN: AZ Insurance, 1260 Energy Lane, St. Paul, MN 55108 with a cashiers check or money order as payment, then schedule by phone at 800-899-4089 The form will be processed within 48 hours from the time received. Allow 4-8 days for mail delivery. Personal checks and/or cash are not accepted.
3. You should arrive at the testing center 15 minutes before your scheduled appointment to allow time for you to sign in, verify your identification and have your photo taken.
4. You must provide a valid form of identification before you may take your exam. The document must be U.S. government-issued (driver's license, state-issued identification card or military identification); it must have a current photo and your signature; and the name on the identification must be the same as the name used to register for the examination (including designations such as "Jr" or "III," etc.).

5. Upon passing the exams you must apply to the Insurance Licensing Section within 120 days. You must submit:
a. Application (Form L-169)
b. Fingerprints on blue-outlined fingerprint card (Form FD-258)
c. Non-refundable fee of $120.00 - 4 year license, renewable quadrennially
d. FBI Fingerprint Processing Fee of $29.00
e. Examination score report attached as the last page of your packet

A. Submit fingerprints ($24 processing fee)
B. File proper application
C. License fee - $60.00 for 2 year license, renewable bi-annually.
